Outbreak Protocol: a lone survivor backs away from a horde of infected pouring through a ruined city street

Image disclaimer: The images on this page are AI-generated artistic renditions of features in the mod, not in-game screenshots. In-game visuals use Minecraft’s own block art style and will look different.

The sun won’t save you.

Outbreak Protocol turns Minecraft’s zombies into an outbreak. They hunt day and night, move in herds and hear every shot you fire. But they have to see you first. So stay low, stay in the dark, and stay quiet.

It comes with:

  • Infected that hear you and have to see you, so stealth actually works 🤫
  • Hordes that answer your noise and dig through your walls 🌊
  • An unseen Director that decides when things get worse, and when you get to breathe 🎚
  • Eleven kinds of infected, seven of them Specials, each with a body all its own 🧟
  • One bite, 24 hours to find a cure 🩸
  • Field Chemistry: brew your own gunpowder, Molotovs, smoke and medicine ⚗
  • The Carrion Strain: four survivors against one Overlord, or everyone against the strain 🧬

🤫 Stay low. Stay in the dark.

A survivor's gunfire sends sound rings down a ruined street while infected turn toward it from every alley

Noise draws them in, but they only come for you once they see you. Crouch in the shadows behind one and you can walk right up to it.

  • Sneak: crouch and they have to get much closer to notice you.
  • Kill the lights: darkness is your best armour. Under the noon sun, you have nowhere to hide.
  • Silence the spotter: one that spots you growls, then screams for its whole herd. You have seconds to drop it.
  • Strike first: your first shot on a zombie that never saw you coming hits twice as hard.

🌊 They come as one

A wave of infected crests a hill toward a torch-lit survivor camp under a glowing red heat bar

No more lone zombies. They roam in herds, and every gunshot, explosion and swing of your pickaxe tells them where you are. Make too much noise and the horde comes. A heat bar on your screen shows how close you are.

Your walls won’t stop them. A horde that can’t reach you digs straight through wood, stone and even iron. Only obsidian holds.

Something is running this outbreak

An unseen AI game master, in the spirit of Left 4 Dead, watches how loud you are and how hard you’re fighting. It builds the pressure, sends the horde, lets the last of them thin out, then gives you a real breather to patch up, reload and loot.

  • It hears you: keep making noise and it answers with a horde. Bigger squads draw bigger hordes.
  • It hates easy: have it too easy for too long and something nasty turns up. You never see it arrive.
  • It never sleeps: stay silent forever? A horde finds you anyway.

🧟 Know what’s coming for you

Four infected in a ruined street: a hooded bike courier, a riot cop in black armour, a pustule-covered predator and a fungus-grown fallen survivor

Every infected has a body all its own, so you can read the threat at a glance.

  • Runner: a turned bike courier. The first to hear you, and the first to arrive.
  • Infected Feral: claws to its knees and glowing pustules. It lunges hard.
  • Outbreak Zombie: a riot cop built like a wall, with the bite most likely to infect you.
  • Undead Survivor: what you become when the clock runs out, still wearing your own armour.

Seven Specials. Never where you see them arrive.

Seven special infected in a burning city: a club-armed Brute, a poncho-wearing Screamer, a red-eyed Stalker, an acid-dripping Spitter, a tongue-whipping Lasher, a swollen Bloater and flaming Death Bats

  • Brute: one arm swollen into a concrete club. When it roars, it charges straight through doors.
  • Screamer: a tiny figure in a yellow poncho. Its third howl brings the horde. Kill it fast.
  • Stalker: two red eyes in the dark. It pounces, pins you and claws.
  • Spitter: its ribcage swings open and acid leaves a toxic puddle behind.
  • Lasher: its tongue grabs you from across the street and reels you in.
  • Bloater: it swells, then bursts. Its bile paints a target on you for every zombie nearby.
  • Death Bat: burning heads on bat wings, hunting in threes. They explode when they die.

🩸 One bite. 24 hours.

A survivor studies a glowing bite on their arm beside a vaccine, an injector and penicillin, under a countdown clock

Get infected and the clock starts. Your legs get heavy, then the sickness hits, then you can barely run. You have one in-game day to find a cure, or you rise as one of them. Milk won’t help. Penicillin buys you time; only the vaccine saves you.

  • Bites that fester: dodged the infection? The bite can still bleed and go bad. Clean it with a Saline Wash or an Antibiotic before it turns. Good armour helps.
  • Bile marks you: get splashed by a Bloater and every zombie nearby comes straight for you.

⚗ Brew your own firepower

Gunpowder isn’t free any more. Dig sulfur from deep underground, cure saltpeter from rotten flesh, and build a Chemistry Station to brew everything you need to survive:

  • Gunpowder
  • Molotovs
  • Smoke grenades they can’t see through
  • Medicine and the cure

⛺ Build a place worth defending

A camp under a Perimeter Beacon's particle dome, with a sleeping bag by the fire

Light a Perimeter Beacon and nothing hostile spawns inside its dome. Catch your breath before the next run. With Legendary Survival Overhaul installed, an Accessories screen lets you wear its rings, necklaces and charms.

🧬 The Carrion Strain

Under a blood-red sky, the pale Carrion Overlord raises a syringe of red serum while his zombie army gathers and four survivors watch from a distant rooftop

One player injects the ampoule and becomes the Overlord, a monster who turns the dead into an army. Four survivors have to hunt them down before the whole world falls.

  • Be the Overlord: your claw infects survivors and recruits zombies. Sneak-claw one into a Brute, Spitter or worse, and feed meat into bubbling Incubators to brew the serum that makes a Patient Zero.
  • The clock only goes up: every Patient Zero alive pushes World Infection higher. At 100% the world falls.
  • Take the heart: the Overlord has ten lives, and a kill only counts if you rip out a heart before they sprint back to their body.

Two Patient Zero bosses

The Orderly, a hulking hospital giant with a glowing maw in its chest, beside The Sower, a bandaged stilt-walker hung with glowing egg sacs, over a lone survivor

The Orderly is a hospital giant that grabs you, lifts you to the maw in its chest and feeds. The Sower is a bandaged stilt-walker lobbing egg sacs that hatch into exploding crawlers. Every hit infects.

The Containment Crypt

Inside the Containment Crypt: a Patient Zero on its dais between broken glass vats, candles everywhere, a crystal beam from an obsidian podium, a hazard-striped hatch in the floor

Each Patient Zero rises in the bio-lab tomb where it was sealed away. The land around it rots crimson, a trapped checkpoint guards the gate, and the hoard waits below the hatch. While its boss lives, nobody can break in or build around it, and it won’t be lured away.

The Wyrm, an infected Ender Dragon of rotting black scales and exposed ribs, dives over a Patient Zero lair while survivors watch from a ruined rooftop

Infect the dragon. Raise every Patient Zero and the Overlord can infect the Ender Dragon itself. It becomes the Wyrm, and a Carrion Crystal ignites over every crypt. The Wyrm can’t be hurt until every crystal is shattered.

No villain? Fight it together. Crush the ampoule instead of injecting it and the strain runs itself: bosses rise on their own, the dragon is infected from the start, and your whole crew is on the same side. Three ways out: the world falls, the survivors take the tenth heart, or the Wyrm dies.

✅ Requirements

  • Minecraft 1.20.1 with Forge 47 or newer. No required dependencies.
  • Works even better with TaCZ guns, The Lost Cities, Legendary Survival Overhaul and the Federation Of Kaos mods below.
  • Install on both the client and the server.

Part of Federation Of Kaos

Outbreak Protocol is the threat in the Federation Of Kaos modpack, a post-apocalypse survival shooter set in ruined Lost Cities. Kaos Protocol guns are loud, so every shot pulls them in. Wire in an Augment Protocol sensor and a minimap shows exactly where every zombie is looking. Quiet Straps on your Carry Pack make you harder to hear, Kaos Missions borrows its hordes, Kaos Hunter puts bounties on every Patient Zero, and Teams lets your squad pick each other up. It runs on its own in any Forge 1.20.1 pack, and you are welcome to include it in yours.
